已解決 表單怎么在前端調(diào)用出來，提交不了

表單怎么在前端調(diào)用出來，表單樣式已經(jīng)有了，我復(fù)制預(yù)覽的表單里面的代碼，把它粘貼到我的模板表單里調(diào)用，結(jié)果測(cè)試提交，提交不了，顯示這個(gè)：
